Configuring the Subscriber Intake Form

Activation Requirement
Subscriber data collection is a platform‑level feature that must be activated by HeyPeers.  Once activated, the Intake Form becomes available for configuration.
The Subscriber Intake Form is the customizable form members complete when subscribing to your organization. It allows you to collect additional information beyond the standard profile data provided by HeyPeers. You can choose which fields to collect, which fields are required, and whether to include survey questions.

How the Intake Form is Structured 

The intake form consists of three sections:
  • Data Fields
  • Survey Questions
  • Disclaimer
These sections appear in this order when members subscribe.

Data Fields Section (Personal Information)

The data fields section is made up of four base fields that are always displayed plus any additional fields your organization chooses to enable.

The four base fields are:
  • Username
  • First name
  • Last name
  • Email
These fields are auto‑populated from the member’s HeyPeers profile and are not removable or configurable (you cannot turn them off or make them optional).

The additional available data fields are:
  • Phone
  • Gender
  • Date of birth
  • Emergency contact name
  • Emergency contact number
  • Ethnicity
  • Country
  • State
  • City
  • Zip code
These fields allow your organization to collect additional demographic, contact, and safety‑related information. Each field can be marked as required, requiring the subscriber to provide data before the form can be submitted.

Subscription Survey Section

After the data fields, the intake form displays any survey questions your organization has added. If no survey questions are added, this section of the intake form is hidden.

Survey questions support several formats:
  • Short answer
  • Yes/No
  • One‑to‑five scale
  • Dropdown

Subscription Disclaimer

At the end of the intake form, subscribers must agree to a disclaimer statement before they can submit the form. This text appears as a checkbox with a required acknowledgment.

By default, the following text is displayed to your subscribers.

By checking this box, I acknowledge that <organization name> virtual support groups are facilitated by trained peer leaders, not mental health professionals. Such sessions are intended to provide participants with emotional support and connection to others with similar lived experiences. But they are not a replacement for therapy provided by or the professional judgment of mental health professionals.

You can edit this text in your organization’s Settings page, (Acknowledge Disclaimer Text field). For detailed instructions, see Organization Settings.

Accessing the Intake Form Settings 

Administrators only

In the Organization Management Module, select Intake Form from the side menu bar.

The Subscriber Intake Form page is displayed. On this page, you can:
  • Add, edit, and delete survey questions
  • Turn on/off data fields; turn on/off "required" for each data field.
  • Preview the Subscriber Intake Form to see what the subscriber sees.

Configuring the Intake Form Sections

Now that you understand how the intake form is structured, the next step is configuring the two customizable sections:
  • Data Form Fields
  • Survey Questions
These settings determine exactly what information your organization collects from subscribers.

1. Data Form Fields

The Data Form Fields section controls which personal information fields appear on the intake form and whether each field is required.
On the Subscriber Intake Form page, select Default Form Fields.
A pop‑up window appears showing all available fields your organization can enable.
In the Subscriber Forms pop-up window:
  • Check a field to display it on the intake form
  • Uncheck a field to hide it
  • Mark “Required” if subscribers must complete the field before submitting the form
These settings allow your organization to collect the information most relevant to your needs.

Select Submit to save your selections. Your updates take effect immediately for new subscribers.

2. Survey Questions

Survey questions appear after the data fields and allow your organization to collect additional, customizable information.

Viewing Existing Questions 

When you open the Subscriber Intake Form page, any existing survey questions are displayed in a table. They are displayed in the same order as they appear on the intake form.

Adding a New Survey Question 

Select +Add a Question.

The New Intake Form Question page is displayed.

The form includes the following fields:
  • Add Question - the text of the question
  • Custom Message - optional text displayed above the question (useful for instructions or grouping related questions)
  • Question Type - choose from:
    - Short answer
    - Yes/No
    - One‑to‑five scale
    - Dropdown
Select Submit to save the question.
How to Add Dropdown Options
If you selected Dropdown as the question type, you will use the Edit Question page to add, edit, or remove dropdown options. Follow the instructions in the Editing a Survey Question section below.

Editing a Survey Question 

To edit an existing question, select Options next to the question. From the dropdown list, select Edit.

The Intake Form Question page is displayed.

The Edit screen allows you to update:
  • Question text
  • Custom message
  • Question type

If the question type is Dropdown, the Edit screen also includes a section where you can:
  • Add new dropdown options - select + Add Option
  • Edit existing options - type directly into the option field
  • Remove options - select the X next to the option

After making your changes, select Save to apply your changes.

Reordering Survey Questions 

To change the order of questions, select Options next to the question you want to move. 
From the dropdown options, choose the direction you wish to move the question. Choose one of the following:
  • Move to top
  • Move up
  • Move down
  • Move to bottom
The order you set here determines the order subscribers see on the intake form.

Deleting a Survey Question 

To delete a question, select Options beside the question. Choose Delete on the dropdown menu.

This permanently removes the question and all associated subscriber responses. This action cannot be undone.

3. Previewing the Intake Form 

Select Preview to view the intake form exactly as a subscriber sees it.

This preview helps you confirm the layout, field order, and survey question flow before subscribers interact with it.
